Lilly Cares is a patient assistance program conducted and administered by Lilly Cares Foundation, Inc., an operating foundation created, funded, and supplied by Eli Lilly and Company. The Lilly Cares program is an outgrowth of Eli Lilly and Company's ongoing efforts to provide ill and needy individuals and infants, who are legal US residents, with access to the Company's medications regardless of their ability to pay. The program was created to offer free medication, through physicians, to eligible patients.
Lilly Cares assists patients who are uninsured and whose incomes are less than 200 percent of the federal poverty level. In 2006, the Lilly Cares program responded to over 158,000 requests, and provided to needy individuals and infants, free of charge, Lilly medications valued at over $155 million.
Most Lilly products are available through the program. Eligibility is based on a patient's inability to pay and the lack of third-party drug payment assistance, like insurance, Medicaid, and government, community, or private payment assistance programs. Individuals eligible for Medicare are not eligible for Lilly Cares services. The Lilly Cares program is now a 12-month enrollment period. Medication orders are shipped for a 4-month supply, unless a lesser amount is requested by the provider. Refills for medication orders are requested with the Fax Refill Request Form, which is enclosed with each medication order.
Patients can download a blank application from the Lilly Cares web site or make arrangements by calling 1-800-545-6962 to receive applications by fax. Applications must be completed and signed by the patient and the physician.
Product will generally arrive at the physician's office 4 weeks after we receive a completed application and the patient is accepted into the program.

Eligibility for the Program
Basic eligibility requirements are as follows:
Patients must be legal US residents.
Patients' income must be below certain levels, based on the number of people in the household.
Patients must not have any other prescription drug coverage (Medicaid, VA, Blue Cross, Medicare Part D, etc.)
Patients must not be eligible for Medicare.
