Lilly Cares is a patient assistance program administered by Lilly Cares Foundation, Inc., a private operating foundation. Lilly Cares Foundation was created by and receives its financial and product support from Eli Lilly and Company. Lilly Cares Foundation and its patient assistance program is an outgrowth of Eli Lilly and Company´s ongoing efforts to provide ill and needy individuals and infants, who are US residents, with access to the Company´s medications.
Lilly Cares assists uninsured patients with incomes less than 200 percent of the federal poverty level. In 2008, the Lilly Cares program assisted more than 142,000 patients, and distributed Lilly medications, valued at over $185 million, to eligible patients free of charge.
Most Lilly products are available through the program. Eligibility is based on a patient's inability to pay and the lack of third-party drug payment assistance, such as insurance, Medicaid, and government, community, or private payment assistance programs. Individuals eligible for Medicare are not eligible to receive medications through Lilly Cares.
The Lilly Cares program has a 12-month enrollment period for eligible patients. Lilly Cares generally ships a 4-month supply of medication unless a lesser amount is requested by the prescriber. All medications are shipped to the prescriber´s office approximately 4 weeks after Lilly Cares receives a completed application and the patient is accepted into the program. The prescriber may request refills for patients using the Fax Refill Request Form, which is enclosed with each medication order.
Patients can download a blank application from the Lilly Cares web site or make arrangements to receive an application by mail or fax by calling 1–800–545–6962. Applications must be completed and signed by the patient and the physician.

Eligibility for the Program
Basic eligibility requirements are as follows:
Patients must be a US resident
Patients´ income must be below certain levels, based on the number of people in the household.
Patients must not have any other prescription drug coverage (Medicaid, VA, Blue Cross, Medicare Part D, etc.)
