If your job was affected by the COVID-19 crisis you may qualify for the Lilly Cares PAP to receive your prescribed Lilly medication for free. Read eligibility details on this website and call us at 1-800-545-6962 with questions. If your proof-of-income document does not reflect your recent change of income, please include a brief letter with your application explaining your situation.

At this time, only submit your application using our online submission process or via fax using the fax number located on the bottom of the application form. Please do not mail your application.

The Lilly Cares Foundation, Inc. ("Lilly Cares") is a nonprofit organization that offers a patient assistance program to help qualifying patients receive Lilly medications at no cost.

Lilly Cares has helped millions of patients in the United States obtain Lilly medications in the areas of mental health, diabetes, growth hormone disorders, men's health, osteoporosis, cancer, psoriasis, arthritis, and migraine through our patient assistance program. It's important to Lilly Cares to make sure that those who can benefit from these medicines have access to them.

The Lilly Cares Foundation does not charge a fee for participation in Lilly Cares. Patients are not required to use a third party who charges a fee to help with enrollment or medication refills. If you use a third party who charges a fee to help with your enrollment or refill of your medication, this money is not paid to The Lilly Cares Foundation.
